CALGB Membership Information

CALGB has three types of membership: main member, at-large member and affiliate. A main member institution may have affiliate members or may be a stand-alone institution. The main member network is expected to have at least 50 registrations annually to CALGB clinical trials by the end of its second year. An at-large member institution may have affiliate members or may be a stand-alone institution. The at-large member network is expected to have at least 30 registrations annually to CALGB clinical trials by the end of its second year. An affiliate member must have sponsorship of a main member or at-large member in order to join the CALGB and is expected to have at least 6 registrations annually to CALGB clinical trials by the end of its second year.

Deadline for membership applications are due in the Central Office two months before the Group Meeting.

Please choose a membership application packet from the list below.
Each packet contains all the material you will need for your membership application except for assurance documents from the Office for Human Research Protections (OHRP).

If you are applying for CALGB institutional membership for a veterans administration hospital, please use the VA At Large/ VA Affiliate Membership Application packet. Everyone else should use the Main Member/ At Large/ Affiliate Membership Application packet.
